Hello all,
  As you have seen, the document has been formally submitted; phew!  I
had indicated on Friday that I would actually submit things first
thing Monday morning, but I won't be in tomorrow morning as it turns
out, so I put things in now.  The only differences from the draft
posted Friday are a revised version of Figure 12, some cleaned-up text
in the SN section thanks to Chris Smith, and revised numbers for
Pan-STARRS in the final chapter, thanks to Nick Kaiser. 

   Thanks again for everyone's hard work on this!  I hope to send out
some specifics on plans for the future in the next week or so.
