Here is a new copy of the form for requesting schedule changes.  The old 
one is now buried far down the list and contains a far out-of-date list of 
institutional schedulers.

This form is generated by either the scheduled observer(s) or PI(s) and
must be emailed to their institution scheduler(s) and all the observers
and PIs involved in the science programs.

In case a scheduler is unavailable (and no one is appointed during their
absence), this form can be sent to Ed, Bruce, or I without the scheduler's
approval, copied to the institution scheduler, and all observers must send
their agreement via e-mail to Ed, Bruce, and I, prior to the start of the
first observing run.

Verbal requests will be taken on a case-by-case basis and only when there
is less than 4-hours before the first observing run.

REQUEST:  state whether the request is a trade, or for open time on the
current schedule, or that an institution wants to release scheduled
observing time back to the community.  If there is some alternate reason
for making the request, please provide a short explanation

INSTITUTIONAL ID NUMBER(S):  include ALL science program number(s) involved
in the request as they appear on the published schedule.  If there is a new
program number, the standard ASCII form for science proposals should
accompany the request

SCHEDULED OBSERVER(S) : list the observer(s) name and institution
affiliation as published in the schedule

CONTACT INFORMATION:  this is very important.  List all contact points for
all observers, i.e. e-mail, home phone, office, phone.    (especially just
prior to the start of the observing run)

PI(S):  list the principal investigator(s) with their contact information
as it appears on the science proposal.  If their contact is different due
to personal schedule, please list their current contact

SCHEDULED OBSERVING DATE/TIME/INSTRUMENT:  list the current
date/time/instrument for the observing runs in the request

NEW OBSERVING DATE/TIME/INSTRUMENT:  list the proposed date/time/instrument
for the observing runs in the request

BRIEF DESCRIPTION:  provide any additional information which helps the
observing specialists prepare for your run.
